I had applied for withdrawal of my pf from my previous employer and have waited for a long period for confirmation of the same. my requested pf withdrawal has not been processed.
following are my pf withdrawal details:

I had applied for withdrawal of my epf pension claim 10c from my previous employer (bhel)and bhel have forwarded it to the assistant commissioner (exam-i),office of the regional p f commissioner, no.20,royapettah high road,chennai 600014 . on 15/07/2010 and provided me a copy of the same.i have registered grievence at epf section many times and different reply came and at last they told non existance of such account. so iam requesting the total history for my pension account.

Information regarding -correction of name in employee public provident fund records

My name has been wrongly entered in your records as ************* in your records. i had given the correct name while applying for the epf account.
correct name : *********** organization: verizon data services chennai
epf account no.: tn******* uan : ******* in spite of repeated requests the name is not getting corrected. i want to know the following:
(1) what was the name as given in my original epf application form ?
(2) what is the name currently appearing in your records ?
(3) how did this discrepancy arise ?
(4) why it has not been corrected even after applying for name correction in the prescribed form through my employer ?
(5) when will it be corrected ?
