Information regarding - status of withdrawal of my employees provident fund (epf) account

I have an employees provident fund (epf) account with my previous employer tata teleservices maharashtra ltd(establishment name) establishment based in mumbai (city). i applied for withdrawal of my epf account on 20 june 2013 , but i have been waiting for almost a year now. i would like to know the following information.
1. copy of inward register showing the date on which my pf/epf claim application (form 10 c) was received by your office.
2. please let me know the balance in my account excluding interest.
3. please let me know the current balance in my account including interest.
4. as of reply date; please inform me the name, designation, phone number and email address of all officer(s) who are involved in processing of pf/epf form 10 c claims.
5. file number of my pf/epf claim application ( form 10 c).
6. certified copy of excerpts of the rules / regulation / notification / order / circular / memo / norms / guidelines / direction, prescribing time frame for processing of form 10 c claims.
7. name and designation of the officer to complain, on non disposal of the application within the time frame.
8. certified copies of all correspondences sent / received for disposal of my application.
9. claim id generated against my claim application.
10. please provide me the details of first appellate authority u/s 7(8) of rti act, 2005 with detailed postal and email address.

I am writing to get help regarding my pf transfer status which was initiated almost 3 years ago. i have not been able to trace the status for account mh/*****/***
post an update in the epfo claim status website in april 2013 stating that the account is settled. i am however unable to find the transfer made to my current pf account which is kn/*****/*****
my old pf account in the malad, kandivali, mumbai office is : mh/92795/196 i had submitted request for transfer to account : kn/*****/*****.
an amount of rs 193299/- vide neft dtd 03.09.2012 has been transfered to members present pf a/c no bg/bng/*****/***** attached is the annexure k.
